Apical thrombus

A 51-year-old gentleman with no significant past medical history presented to the emergency department with progressive dyspnea and lower extremity edema. Transthoracic echocardiogram showed severely reduced left ventricular systolic function with ejection fraction of 15% and a large left ventricular apical thrombus.
