High Degree AV Block

Description

ECG of a 76 year old woman who was taking four medications with AV nodal blocking properties at the time of admission. Underlying rhythm is normal sinus rhythm at a rate of approximately 80/min. The second atrial wave is a premature atrial complex that arises from a low atrial focus, resulting in negative deflections in the inferior leads. High degree AV block and a junctional escape rhythm at a rate of approximately 40/min are present. The second, third and eighth QRS complexes appear to follow AV nodal conduction as shown by a similar PR interval and shortening of the R-R interval.
