Ectopic Atrial Rhythm

Description

Negative P waves in the inferior leads (II, III, aVF) and short PR intervals (less than 120 msec) are consistent with an ectopic atrial rhythm. A focus in the low right atrium would result in inferiorly to superiorly directed atrial depolarization, and the short PR interval is due to shorter distance that the electrical impulse must travel from the ectopic focus to the AV node. The rate is approximately 60/min in this ECG.

This ECG from the same patient shows upright P waves in the inferior leads, which are consistent with an origin of the electrical impulse in the SA node with resultant superior to inferiorly directed atrial depolarization. This sinus rate is approximately 60/min in this ECG.
