Atrial Tachycardia with 2:1 AV Conduction

Description

Atrial tachycardia at a rate of 148/min is present. The first two atrial waves are conducted to the ventricles. The longer isoelectric segment following the second atrial wave is due to AV nodal conduction during the relative refractory period of the AV node. Following these two initial conducted atrial complexes, 2:1 AV conduction is present, resulting in a ventricular rate of approximately 75/min. Atrial waves are best seen in lead V1. An additional finding is nonspecific ST segment depression in leads V3-6.
