Emphysematous pyelonephritis in a middle aged male

Description

A 49-year old male with known type 2 diabetes mellitus, on insulin therapy, presented with a history of fever, chills and altered sensorium of 2 days duration. He also gave a history of left loin pain of the same duration. Ultrasound of the abdomen was suggestive of left nephromegaly with perinephric fat stranding. NCCT KUB (Figure above)showed foci of air within the left kidney with changes of pyelonephritis. A diagnosis of emphysematous pyelonephritis was made. Broad spectrum antibiotics and intravenous fluids were initiated. Pigtail drainage of the collection was done.
